[Exim] Delivery problem

Hi all, from one month I'm running the latest version of Exim, untill today
the server (Linux SUSE 6.4) has worked very good, but yesterday there was a
loop and the same e-mail was sent to the list about 30 times ...

In the mailer log I have these entries:

Mainlog:

/var/spool/exim/db/wait-remote_smtp.lockfile: timed out

/var/spool/exim/input//13yy8q-0006UM-00-D

This error was generated by a normal e-mail sended by a user to one of our
mail lists (we are using Smartlist as list server)

Any good idea on what's worng on our server ?
