Actually since his host is relay.mydomain.com at pref 20, when exim
builds the MX list it will strip out all the entries with pref values >
= 20, leaving just:-

One way of producing the effect I think he was complaining of would be
to set
hosts_treat_as_local = mail.123.com
However this problem seems rather confused and a concise statement of
the actual problem and the config generating it would help somewhat.
