Check the permissions on /var/mail
On our setup we have
drwxrwxr-x 3 root mail 2048 Jun 29 06:00 /var/mail
.. ie. /var/mail is writable by group mail. Then make sure that whatever
user Exim runs as, is in group mail as well. You'll need to set the group
of all your user agents (pine/elm etc) to be mail too, and set them to be
set-gid.
The alternative which people tend to do, but which I don't really like, is
to make /var/mail have the same perms as /tmp - ie a+rwx,u+t
