RTFM: received_headers_max

When a message is to be delivered, the number of `Received:' headers is
counted, and if it is greater than this parameter, a mail loop is
assumed to have occurred, the delivery is abandoned, and an error
message is generated. This applies to both local and remote deliveries.
Earlier versions of Exim did this test only for remote deliveries, but
because local deliveries (as Exim sees them) may in fact still cause a
message to be transported to a remote host, it was changed.
