[Exim] 421 Too many concurrent SMTP connections and logging

421 Too many concurrent SMTP connections ..

Today I found out after a user complained that smtp_accept_max was set
too low on the outgoing SMTP gateway, so I tuned it up. However
nothing was found in the logfiles about this - shouldn't it be logged
in "rejectlog" ?

Likewise if a host triggers smtp_accept_max_per_host ?
