That suggests that it is not seeing the MySQL header files. Have you got
something like
LOOKUP_INCLUDE=-I/opt/local/mysql/include
in your Local/Makefile? You might also need something like
LOOKUP_LIBS=-L/opt/local/mysql/lib -lmysqlclient
to pick up the runtime library.
