Did those two messages really appear on your log in that order? I would
have expected them to be the other way round.

. The one that says "recipients refused from" is output at the start of
an SMTP connection, when Exim discovers that the sender is in an RBL,
and decides to reject all recipients. It does not, however, preserve
the name of the RBL domain.

. The one that says "recipient <xxx> refused" is output only if
log_refused_recipients is set (it is not set by default) and it
happens when a RCPT command gets rejected. This was a later addition
to Exim as a result of a user request.
