The first delivery into mailman isn't being shown in the exim logs -
might be worth turning up your log level.
I guess the first delivery must have either hung or come back with a
fail code - check what exim should log for either of those.
I have seen a few multiple deliveries with exim and mailman, but in
those cases it was *definitely* mailman which sent one delivery reuqest
set more than once.
