[Exim] Comment wanted: extending appendfile

As a result of receiving some patches for supporting MMDF mailboxes,
I've been thinking about extending the appendfile transport in a more
general way, and I'd like some feedback on the ideas. Rather than send
it all to the whole list, I've put a description in
