Have you remembered to make Exim setuid root? If no, do so and try
again. If not:
Did you define EXIM_UID in your Local/Makefile? What output do you get
from
exim -bP exim_user exim_group
What are the permissions on
/usr
/usr/exim
/usr/exim/spool
/usr/exim/spool/input
?
