strings `which exim` | fgrep hosts.allow
will check that a wrapped version of exim is in use and which
allow file it's looking at.

Theres nothing about exim in the hosts.allow ?
Is the hosts.deny file readable by whatever user you're running exim as ?
You could try running the exim daemon with -d<number> and see what
it thinks is happening.
