You might want to read the code in src/transports/appendfile.c from the exim
installation. The procedure that Exim has to take to lock mbox files
(locally and over NFS) is extremely well commented there, and points out
many of the shortfalls that mbox-over-NFS has.
If you have to use NFS for mail and have the option to use Maildir (which
Exim supports), go for it. The only problem is that there aren't all that
many POP3/IMAP4 daemons out there that can read Maildir.
