[EXIM] Found it.

Re my earlier messages about STDERR and STDOUT, I believe I have found
the culprit:

I was still using bulk_mailer. Having changed the alias to
:include:/path/to/list, the problem disappeared. Sorry for the
confusion, and thanks to Andromeda for trying to figure it all out.
