What do people think about removing infected attachments, adding an
appropriate message and delivering the mail as normal, possibly sending
a message to the sender too ?

It violates the principle of sending a message intact, and the RFCs may
have something to say, but if the attachment is just an illustration
to a (long?) piece of text, the main content of the message is
communicated.
Since this would be more work than just failing the mail, I'd like to be
sure it would be legal sensible and appreciated before asking anyone to
code it.
