That certainly looks like a permissions problem. You *have* got Exim
setup as setuid root, I trust? Is it delivering other mail normally?

I put in a bit about spool permissions and local delivery; nothing about
"failed to create spool file" yet, but I'll add something about that -
and the resolution of this problem, when it comes.
