I have md working with exim, but not exactly with the set-up described.
(I have set up nothing special, and am using the almost the same set-up
that I had under sendmail, except that I've removed "bulk_mailer").

I believe that the argument to -f should just be the local-part of
an address, but there is nothing in the docs that actually say that.
Also, make sure that the majordomo user is among the trusted_users
in the main part of the configuration.
Anyway, these are stabs in the dark. Your problem might be something else
altogether.
