If majordomo would fail, it would log something before failing.

Unfortunately not. I'd replace $sendmail_command with something that
makes you see the actual parameters and the stdin that is presented to
the exim call.
Just a thought, your exim actually _is_ in /usr/sbin/exim? That would
look like a quite strange location IMHO.
