Thanks -- this appears to have solved the problem for me.

I was running ntpdate then runq in the script which brings up the ppp
connection to my ISP. Checking back through the logs, it appears
my PC's clock has been mostly running slow, so ntpdate warped time
forward, causing no ill effects on exim's view of message IDs. On the
occasions when the "failed to create" errors occurred, the clock had
been running fast and ntpdate had warped time backwards -- normally
only by a few seconds, but enough to upset exim. :-(

Can I request that this patch is made part of the released version,
please? I'm sure I'm not the only person who adjusts their computer's
clock over a dial-up link, when mail is being received.
