Ah! In the current version of exim, if you have "return_output" set on a
pipe transport, as you do, then if the pipe generates *any* output text,
the delivery is considered to have failed, and the text is reported to
the sender of the message as a delivery failure. That's what's happened
here. The return code isn't relevant. The manual isn't entirely clear
about the precedence here (existence of output takes precedence over
return code); I have made a note to improve the next edition.

There is also an item on the Wish List to provide some more flexibility
in this area, for example, to allow you to say "return output only if
return code is a failure code". I'm hoping to get something implemented
for the next release.
