After a bit more testing and consultation with Philip, it turns out that
this was caused by mutt, the mail client program I'm using. It was
turning off the signals that exim was waiting on.
I fixed mutt to stop doing this but Philip says he will also fix exim to
turn on signals so it's more resistant to this unfriendly behaviour.
