I support this clarification... Indeed this problem caused me to lose much of a
day trying to work out why my rate limiting wasn't working, only to eventually
discover that by using the default "leaky" counting the computed rate
rarely/never exceeded the threshold so my delays never kicked in.
With clearer documentation as suggested I'd have picked up on this sooner.
