I agree with Simon. If the RFCs demand that you send a HELO/EHLO first, then
Exim should require it by default. After all, Exim doesn't let you send a RCPT
before sending MAIL, so why is this any different?

There is no special Exim configuration to make it react like that, it just
does, because the RFCs mandate it to do so...

However, I'm not 100% familiar with the RFCs for all three of SMTP, ESMTP *and*
LMTP so is there ever a case that a "MAIL" is allowed to be sent without a
HELO/EHLO being sent first? If there is, then I completely change my position
on the matter.
