The listening daemon reads the Exim configuration once, at startup.
However, if an option refers to another file, then *that* file is read
every time it is needed. So you could say

smtp_reserve_hosts = /some/file

if you want it to read the file each time it processes the option. If
the number of entries in the file is large, you could use a DBM or cdb
file for efficiency. Some people use databases...
