No more of a problem than 200 GB worth of IMAP traffic stored in the fs ..
...IF you are using an appropriate RDBMS backend.

But an external MTA is needed in any case - and that MTA doesn't need 'SQL
integration' at all.

Even use of the DB for smtp auth is (or can be) separate from DBMail's use for
storage & POP/IMAP auth.
