If that is in a RCPT or MIME ACL, Exim will only add that header once,
however many recipients there are. Amazingly, I actually thought of this
possibility. (Provided that "something" doesn't change for each
recipient.) All the other ACLs can only be run once per message, so
there's no possibility of repetition. If you would RTFM you would find
that this is all explained.
