I am using the startup script that came with cpanel. Its strange that it
only starts up two when I add local_interfaces to the config.

Oops :) I wasn't even paying attention to the port numbers. I saw the
0.0.0.0 and saw the beginning of localhost and just assumed. It is 2:30am
for me after all!
Seeing your posted info from netstat - exactly how are you starting Exim?
Script, or calling exim directly?
