Yes, this is possible.
As an example:

Router:
gdnsmanualroute:
driver = accept
domains = +gdns_manualroute_doms
transport = special_manual
ignore_target_hosts = 0.0.0.0 : 127.0.0.0/8

Transport:
special_manual:
driver = smtp
port = ${lookup mysql {MYSQL_RPORT}{$value}}
