The initial router I have to get around this problem is as follows
dynamic_router:
driver = manualroute
domains = /etc/exim/isps
route_list = * <SMARTHOST ADDRESS>:<YOUR SERVER>
transport = remote_smtp
Keeping the list of ISPs who block Dynamic IPs in a flat text file saves
having to HUP exim each time a new one is added.
