IMHO, go for MailScanner.
In the past I've used combinations of exiscan and sa-exim to allow virus
scanning and spam assassin integration.
Then I found MailScanner which makes everything far simpler (ie
unpatched exim) for us. Support on the MailScanner list is excellent as
is the development cycle. There are also lots of additional features in
MS that I've found useful.
I wouldn't be without it
