You caught me, I am using exiscan-ACL with exim4
I want to add another Header similar to what happens to when Spam
Assassin scans.
X-Virus-Scanned: blahblahblah...
I have tried some conditional stuf first_delivery and other things in
combination with the scanning.
I am just missing it. I have read through quite a bit of
Documentation...

Chapter 10: Message Filtering in the Exim4 book
Many examples online and from older versions with "updates" by me
applied.
Howtos for even Q-mail and how it use inject to do things and try to get
exim to mimic that...
*I* just want clamd with a client like clamdmail to write headers like
spamd/spamc do for Spam.
Like I said, I am just missing it.
