At Exim 4.X you can make it easy with system_filter.
Insert in /etc/exim4/conf.d/main/02_exim4-config_options:
system_filter = /your/path/my_filter
system_filter_user = mail
And in the /my/path/my_filter insert the next lines:
if first_delivery and
$sender_address matches "\\Ntom@mydom\\.org\\N"
then
unseen deliver tom@???
endif
