Next time, try

exim -d ..... 2>/some/file

so the standard error is sent to a file.

Have you tried replacing the transport filter with a program or script
that just copies input to output, without trying to write to a debug
file?

Is the transport filter relying on anything in the environment, such as
PATH or other environment variables? They won't be set.
