The Sender: is being set to usercode@primary_domain because the usercode
is not trusted to be able to set arbitrary From: addresses.

You might want to read up on "trusted_users" (and/or "trusted_groups").
It's explained well in The Exim Specification, section 5.2, "Trusted and
admin users".
